To schedule an appointment, please call us at (316) 312-0002, or use our secure appointment request form below to request an appointment online. We will respond to your request within 48 hours. Please do not use this form if you have an urgent medical problem or to re-schedule an existing appointment.

Prior to your first appointment we need to gather some important information from you in order for the visit to run more smoothly and focus on you rather than inputting.

We recommend that you download and print the applicable patient forms below, complete them and fax them to us at (316) 440-3200 prior to scheduling your appointment. We can also mail the new patient packet to you with a self-addressed stamped envelope for your convenience. During your initial evaluation over the telephone, the physician may instruct you to print out and complete some of the forms listed to the right, prior to our arrival.
